An antidepressant effect of Tadalafil 5mg once daily in male LUTS and ED

Objective; PDE5 inhibitor has effects of smooth muscle relaxation, as well as an antioxidant function and an endothelial cell protection, which may allow beneficial effects for men with lower urinary tract symptoms (LUTS) and erectile dysfunction (ED). In present study, we examined retrospective data in order to confirm the effect of Tadalafil 5mg once daily administration for male LUTS and ED. In addition, we aimed to clarify its influence against depressive mood.

Material and Method; We retrospectively reviewed 53 consecutive patients who had LUTS and ED. Patients took once daily Tadalafil 5mg at our institution. Total 41 patients who had complete record at least 6 months of follow-up were examined. We evaluated its efficacy using several questionnaires, including AMS score, SHIM, self-rating depression scale (SDS), IPSS, QOL score, and OABSS. Wilcoxon signed-rank test was utilized to assess changes of aforementioned scales of questionnaire from baseline to the last follow-up. As well as, blood test such as PSA (prostate specific antigen), total testosterone, luteinizing hormone, and cortisol were conducted.

Result; The mean age was 58.7 years with mean follow-up of 17.2 months. At 6 months after treatment, 87% of patients represented increased morning erection. When comparing pre- and post-treatment questionaries’ scores, it was found that improvements in SHIM score (P < 0.001), IPSS (P = 0.003), QOL score (P < 0.001), number of nocturia (P < 0.001), and SDS (P = 0.028). Whereas, significant differences were not found in AMS score, OABSS, PSA level, and aforementioned hormonal exam between pre- and post- treatment.

Conclusion; Once daily PDE5 inhibitor use may enhance men's health via increased erectile and urinary function, which might lead to reduce depressive mood. Although more detailed mechanisms need to be investigated, potential antidepressant effects of daily PDE5i for men with LUTS and ED were confirmed.
