Safety and efficacy of high intensity focused ultrasound treatment for vagina-3 months results-

Objective(s): High Intensity Focused Ultrasound (HIFU) technology is used for the treatment of prostate cancer and liver cancer, uterine leiomyoma and cosmetic surgery ( Face lifting, Slimming) in recent years. It is effective to deeper layer of skin which cannot reach the energy with Radio Frequency and Laser. Ultra Vera® is the vaginoplasty device with HIFU technology which is same technology as Doublo®, HIFU treatment device for the face lifting. We are assessing the effectiveness and perioperative safety of HIFU vaginoplasty in a day surgery clinic.

Material and Method(s): We prospectively studied 29 patients, median age 46±7, between Apr. 2016 and Aug.2017 at LUNA Pelvic Floor Total Support Clinic. Applying ultrasound gel to a cartridge, doctors insert the cartridge to the vagina and irradiates HIFU energy on the circumference. 2 rounds to the inner part and introitus. Average treatment time was 20 minutes without anesthesia Follow up was performed at 3 month. We checked preoperative safety, vaginal tightness and sexual satifaction using 5 grade VAS scale , FSDS-R, FSFI, Vaginal Health Index before operation and after operation 3 month. This study was approved by the Ethics Committees of the Yokohama Motomachi Women’s Clinic LUNA in 2016. Written informed consent was obtained from all patients.

Result(s): There was no pain complain without anesthesia during and after operation. No major side effect was recognized. Median 5 grade VAS scale of vaginal tightness improved from 2±0.9 (Median ± Stdev.p) to 3±1.2 .There is statistic significance between them .(p <0.01) Moreovr  FSDS-R, and Vaginal Health Index were improved from 10.5 ±11.7 to 3.5 ±7.2, from 18 ±2.6 to 22 ±3.1. There are statistic significance between them .(both p <0.01) But FSFI and VAS scale of sexual satisfaction level were not improved.

Conclusion(s): HIFU vaginoplasty has a chance to contribute to QOL improvement for women who suffer from pelvic floor disorder . But there may be no relation between vaginal health and sexual health.
